MINIMUM REQUIRED SWIPE CARD DATA FIELDS

Definition = Customer's Social Security number.
Field Parameters = 9 digits with no spaces or dashes. Required field.

An empty check box = 0= No. Not required. Strictly defined Ethnicity requires a check for “Hispanic or Latino.”
Race Not an additional field in OSOS. See ethnicity above. Strictly defined, this field consists of the remaining six choices from above A check =1=Yes. An empty check box = 0= No.
